| Name | Unique name of template. |
| Template Type | Service group type to which this template can be applied. |
| Start Frequency | Starting range of the frequency list. A default value is added on selecting the template type. |
| Frequency Count | Range of frequencies that service groups from this template will use. |
| Annex Mode | DOCSIS Annex for the channel. The Annex is used to determine the channel width; that is, the spacing between two adjacent channel frequencies. |
| Frequency | Frequencies that can be applied when the template is used on a service group. |
| Modulation | RF modulation associated with the frequency. |
