- Click Fault > Fault Name Report. The Fault Name Report window is displayed.
- Click the Show/Hide toggle to show or hide the filter criteria panel (below).

- Enter or select the criteria to specify a managed object in the report:
Group*
IP Address*
Object Name*
To include SDM Internal Traps, select the appropriate checkbox.
* Indicates a parameter entry box that supports entry of more than one value. When multiple values are entered, they must be separated by commas.
- Select Firmware(s) from the scrollable list box.
- Select the Fault Attributes to be included in the report:
Fault Name*
Failure Object*
Fault Type (Event/Alarm, Event, or Alarm)
Description
Category*
Severity

- Click Display. The Fault Name Report with fault name definitions page is displayed:

- The display fault name report can be further filtered by column headers.
- From the list box adjacent to the Display button, select the column headers to select columns to show or hide.
- Click Show Selected Columns to view the selected columns, or click Hide Selected Columns to hide the columns.
- Columns that can be selected include:

The name of the fault
Description
The description of the fault definition
ITU Category
The ITU category for the fault; for example, Operational, Communications,Quality of Service, and so on
Severity
The severity of the fault; for example, Critical, Info, Clear, Derived
Is Alarm
The alarm state (Yes [in alarm] / No [not in alarm])
Failure Object
The failure object of the fault (trap)
Firmware Version
The column heading that displays the firmware type, model name, and version information for which fault information is shown; for example, OM, 2000, 1.1.4 in a column heading indicates fault information shown in that column is for an OM 2000 Out-of-band Modulator running version 1.1.4 firmware. In that column, an X appears in a row if that type of fault exists for the device
